Please return this form with your washed baby clothes and payment (50% now, 50% prior to shipment of finished quilt.)

I will use as much of each piece of clothing as possible. If I don't use a piece, I will return it with the quilt. I do not return remnants/ pieces of outfits. Additional fabrics may be added if necessary.

If you live in the Des Moines, Iowa Metro Area, we can arrange to meet for the pickup and delivery of the clothes/quilt, so do not include shipping.

	Standard Quilt: 40" x 54" - Typically requires about 20 onsies/pajamas/outfits	$150
	Twin Sized Quilt: 63" x 87" - Requires about 40 onsies/pajamas/outfits	$300
	Small Custom Wall Hanging: roughly 20" x 20" - 8-12 pieces of clothing	$75
	Medium Custom Wall Hanging: roughly 30" x 30" - 12-20 pieces of clothing	$100
